Being in network does not mean every procedure is covered. Two plans from the same carrier can cover the same procedure differently, so we verify your specific benefits before your first visit.

Some plans go by more than one name. If you are not sure whether yours is on this list, call us with your insurance card to hand and we will check it, tell you what your visit is likely to cost, and flag anything that needs prior authorization.

We also see self-pay patients and will give you a written estimate before anything is scheduled.

Verify Coverage

We check your insurance benefits before your appointment to understand your coverage and out-of-pocket costs.

Service Provided

You are seen by Dr. Steinberger at your scheduled visit, and the plan is set that day.

Claim Submitted

Our billing team submits a claim to your insurance company with all necessary documentation and procedure codes.

You Pay Remainder

After insurance processes the claim, you receive an Explanation of Benefits (EOB) and a statement for any remaining balance.

Understanding Your Costs

Co-Pay

A fixed amount you pay at each visit. The amount is set by your plan, and we will tell you what yours is when we verify your benefits.

Deductible

The amount you pay before insurance starts covering services, reset annually.

Coinsurance

Your share of costs after meeting your deductible, usually expressed as a percentage like 20%.

EOB (Explanation of Benefits)

A statement from your insurer showing what was charged, what they paid, and what you owe.

We accept cash, checks, and all major credit cards. Payment is expected at the time of service unless prior arrangements have been made.

No Surprises Act: You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services before those services are provided.

Do you accept my insurance?
We are in network with Ambetter, Cigna Commercial, Cigna Medicare, Devoted Health, Florida Community Care, Florida Medicaid, Humana, Medicare, Oscar, Preferred Care Network, Preferred Care Partners, Solis, Sunshine Health and Wellcare, and only with these plans. What a plan covers still varies by policy and by procedure, so we verify your benefits before your first appointment. If you are not sure whether your plan is one of these, call our office.
What if I don't have insurance?
We welcome self-pay patients and offer transparent pricing for our services. Payment is expected at the time of service, and we accept cash, checks, and all major credit cards. We can also discuss payment plan options for larger procedures to make care more accessible.
What are co-pays and deductibles?
A co-pay is a fixed amount you pay for a covered service, usually at the time of your visit. A deductible is the amount you must pay out-of-pocket for covered services before your insurance begins to pay. After meeting your deductible, you typically pay coinsurance (a percentage of the cost) until you reach your out-of-pocket maximum.
Will I receive a bill after my appointment?
After we submit your claim, your insurance company will send you an Explanation of Benefits (EOB) showing what they covered and what you owe. Once we receive payment from your insurance, we will send you a statement for any remaining balance such as co-pays, deductibles, or non-covered services.
What is the No Surprises Act?
The No Surprises Act protects you from surprise medical bills. You have the right to receive a Good Faith Estimate explaining how much your care will cost before you receive services. If you're uninsured or choose not to use insurance, you can request this estimate in advance so there are no unexpected charges.
